Format for your report

Please follow this standard order and format in your email:

(We developed the list of questions with our contacts -- so that they get all the information they need to quickly start investigating your case.)

Problem statement: [In one or two sentences, summarize the problem.]

Details -- so that they can investigate your case and contact you:

- Name (exactly as the Benefits Center or Alight Retiree Health Solutions has it.)

- ZIP Code.

- Phone number.

- Email address.

- Which HP company (Hewlett-Packard or Hewlett Packard Enterprise) or predecessor (such as Compaq, DEC, EDS, Nimble) you last worked for.

- Which phone center you are working with -- HP Inc Benefits Center, HPE Benefits Center, or Alight Retiree Health Solutions. Please specify the phone number you have been calling.

- How you left that company: WFR, WFR-to-EER, plain-EER, Disability, or...

- Were you covered by another employer after you left HP?

- Are you on the "Pre-2003" HP Retiree Medical Program? Note: Depending on age, years of service, and other factors, you may qualify for the "Pre-2003 HP Retiree Medical Program" -- as opposed to the current "HP Retiree Medical Program." ("2003" refers to the year when HP implemented the program, not necessarily your year of retirement.) Details: hpalumni.org/Pre2003

Which Medicare status?

- Medicare-eligible.

- Not yet Medicare-eligible.

- Mixed (Mix of Medicare-eligible and non-Medicare-eligible family members.)

If a health plan is involved in the issue, name of the health plan.

If Alight Retiree Health Solutions (formerly Aon Retiree Health Exchange) is involved in the issue, please add these details:

- State of residence. (Agents are required by law to be licensed to sell health insurance in your state.)

- Did you try using ARHS's website to find plan details? [HPInc https://retiree.alight.com/hp  HPE https://retiree.alight.com/hewlettpackardenterprise ]

- Alight Retiree Health Solutions insurance agent's name, if known.

Brief description, step-by-step in chronological order, of what specifically happened. Be sure to include the date and approximate time when you last talked to the Benefits Center on this issue, with the person's name if possible.

Advice on content:

- Do not include confidential information such as SSN or employee number.

- To ensure that the case is clearly understood, leave out any extraneous material

- Please list the events in chronological order.

- Do not include attachments.


Put your note in the body of an email. (Due to the flood of phishing attacks, email systems at companies tend to block -- or severely delay -- emails that originate outside the company and contain attachments.)
